Annual birth month reviews Posted or Updated on 5 Jan 2023

Patients who have any of the following conditions are encouraged to call the surgery ahead of their birth month to arrange their annual birth month review:

• Diabetes
• Chronic Heart Disease
• Chronic Kidney Disease
• Thyroid
• Hypertension
• Asthma
• COPD

Please note, if you do not have any of these conditions, there is no need call the surgery as we do not do routine annual reviews if they are not required. If you feel that you need a review for a condition not noted above, you would need to arrange a GP appointment to discuss further.
